Go to store .
Click on profile.
Open dashboard.
Now choose your app.

Now click on your app and send screenshot of app status.
Or check app status and if it is unpublished then set it to published by clicking on it.

Change status to published.

I also forget to do that.
But after spending few hours I found the problem.
Most of the new users forget about changing app status to published.

one more question does the banner need to be 1024x500 or can is be smaller?

1024*500 is min size.
You can use any image also.Just choose an image and resize using Paints.
